On the page with a detailed description of the desired product there is a unique item number for each distinct size. Make a note of the item number for reference when completing the order form.
Ordering
Click on “Order Form”. The order form is self-explanatory. If you are in the state of Colorado, be sure to include your sales tax exemption number. Orders in Colorado that are not accompanied by a sales tax exemption number may be billed for appropriate sales tax(es). Completed orders may be submitted via fax to 1-719-347-2678 or as an attachment to an e-mail sent to:

Shipping and Handling
Equipment orders in the United States are delivered via U.S. Postal Service,
United Parcel Service (UPS), Federal Express, or other freight company. Shipping
and handling charges are based on actual cost and depend on size, weight, and
destination. If shipping and handling charges exceed 10%, APA will contact the
customer before processing the order.
Payment and Billing
Customers with an established account will be invoiced within 3-5 days of an
order being processed. All other customers should call APA to set up an account
or arrange to pay by Master Card or VISA.
Return and Refund Policy
You must contact APA by phone (1-719-347-2668 or toll-free 1-866-347-2668) before returning any item for replacement or refund. Some items, if they are removed from packaging and placed with a patient, even for trial purposes, are subject to infection control regulations and may not be returned for replacement or a refund. APA will provide the customer with return shipping instructions for any item approved for return. If the return is approved for a refund, shipping costs and a 20% restocking fee will be deducted from the refund amount due.
